Comprehensive Psychiatric Evaluation

60 minutes – $350

A comprehensive evaluation of mental health concerns, including personal background, psychiatric history, and current symptoms. The session focuses on developing an individualized treatment plan, reviewing medication options, and offering referrals for further evaluation or specialized services when appropriate.

Follow-Up Psychiatric Appointment

30 minutes – $175

A follow-up session focused on ongoing care. This visit includes reviewing symptoms, monitoring treatment response, managing medications, and providing support to track progress and make adjustments for overall well-being.

GeneSight Testing

up to $330

GeneSight testing helps guide personalized medication decisions by analyzing how genes may affect response and side effects. The fee includes sample collection, results review, and recommendations. Costs billed by GeneSight may be up to $330 depending on your insurance coverage.

In-House Call Service for Assisted Living Facilities

60 minutes – $400
30 minutes -$200

I offer an in-person psychiatric evaluations and medication management for residents of assisted living and adult care facilities in Nassau County. Visits are coordinated directly with facility staff to provide convenient on-site care and ensure continuity of treatment.

Appointment Cancellation / No-Show Fee

$100

Appointments canceled with less than 24 hours’ notice, or missed appointments, will incur a $100 cancellation fee.
This policy helps maintain availability for other patients and supports timely access to care.

Forms Completion

$75 and up

Completion of medical or mental health forms for school, work, or insurance. Available to patients in care for at least 6 months. Standard forms $75; complex forms may require an additional fee. Not typically covered by insurance.

Insurance Plans Accepted

I am currently in network with select insurance plans and am in the process of credentialing with additional providers. Please contact me directly to confirm whether your insurance is accepted.
For those using out-of-network benefits, a detailed superbill is available which you may submit to your insurance provider for reimbursement.

Your Right to Cost Transparency

Good Faith Estimate

If you are uninsured or choose not to use insurance, you have the right to receive a Good Faith Estimate of expected costs before your services. This estimate helps you understand and plan for treatment without surprise bills, in line with the No Surprises Act.

What Is a Good Faith Estimate?

Under the No Surprises Act, uninsured and self-pay patients have the right to receive a Good Faith Estimate of expected costs before care. This helps you:
– Understand potential costs in advance
– Plan for your care
– Avoid unexpected bills

When To Request One

You may request a written Good Faith Estimate:
– At least 1 business day before your scheduled appointment
– For non-emergency services such as appointments, labs, or medications
– With no obligation to move forward with care

Your Rights

If your final bill is $400 or more above the estimate, you can file a formal dispute.
Disputes must be submitted within 120 days of receiving your bill.
A small administrative fee may apply.

Save a Copy

Keep a copy or screenshot of your Good Faith Estimate. You may need it if you:
-Have questions later.
-Decide to dispute a charge
