Care Coordination
Care Coordination Services in Long Island and Across New York
At InnerMind Psychiatry & Wellness, I believe that effective mental health care extends beyond medication and therapy alone. True healing often requires communication and collaboration among the professionals who support your overall well-being. Care coordination ensures that everyone involved in your treatment is working together toward the same goal – helping you feel better and live a more balanced life.
What Is Care Coordination?
Care coordination is a structured process that brings together all members of your healthcare team so that your treatment remains organized, consistent, and focused on your needs. This may include communication with your primary care provider, therapist, nutritionist, school counselor, or other specialists who play a role in your health.
By sharing relevant updates, medication changes, and treatment goals, I help create a clear and unified plan of care. This integrated approach allows for early identification of potential issues, improved medication safety, and more effective long-term outcomes.
Why Care Coordination Matters
Many individuals see multiple providers for physical and mental health concerns. Without proper communication, it can be easy for important information to become lost or duplicated. Care coordination helps prevent that by ensuring your providers are on the same page.
Some of the key benefits include:
- Improved safety and consistency with medication management
- Reduced duplication of care or conflicting recommendations
- Enhanced understanding of your mental and physical health connection
- Stronger collaboration between psychiatry, therapy, and medical care
- Smoother transitions when changing providers or levels of care
When your providers collaborate, it creates a more holistic picture of your health and allows for better-informed decisions about your treatment plan.

What Care Coordination Involves
As part of your psychiatric care, I obtain your consent before communicating with other providers. With your permission, I may reach out to your primary care physician to discuss medication interactions, lab results, or physical health concerns that may influence your mental well-being.
If you are engaged in psychotherapy, I collaborate with your therapist to align medication goals with therapeutic progress. This ensures that both forms of treatment work together effectively. For clients who require specialty referrals – such as for sleep studies, neurology, endocrinology, or substance use programs – I can assist in coordinating these connections and sharing necessary documentation to streamline care.
When needed, I also support communication with schools or academic counselors for younger patients who may require educational accommodations or ADHD support.
My Approach to Collaboration
My approach to care coordination is personalized and respectful. Every client’s situation is different, so I take time to understand your preferences and determine what level of communication you are comfortable with. I value transparency and will always discuss what information is being shared and with whom.
I view care coordination as an extension of compassionate, high-quality care. My role is to ensure that the professionals involved in your treatment have the information they need to support your progress while maintaining your privacy and confidentiality in accordance with HIPAA regulations.
Virtual and In-Person Care Options
Care coordination is available as part of both telepsychiatry and in-person appointments. Through secure telehealth, I provide psychiatric care and coordination services across New York State, including Long Island, Nassau County, and Suffolk County. For those who prefer face-to-face visits, limited office hours are available in Glen Head, NY.
Whether virtual or in person, the goal is the same — to provide seamless, collaborative care that helps you move toward lasting stability and improved quality of life.
